How can I file the General Form 14 to dismiss a pending family law case?

Q: Recently I got a notice of motion that I have to fill out to dismiss a case because I don’t reside in Yonkers anymore, I live in CT, and I want to know how can I file it out! Thank you!

A: The form you’re referring to is merely a notice. You’d also need to complete an affirmation in support of the notice & attach any applicable exhibits to the motion, along with an affirmation of service. Filings may be done at the clerk’s office or via the Court’s EDDS system. That said, I wouldn’t recommend you proceed pro se & thus you should (at the least) have your papers reviewed by an attorney.
