Q: Hello, my spouse and I are filing an uncontested divorce in NY. We filed out the uncontested divorce forms and will file them tomorrow. We have a question about the Certificate of Dissolution of Marriage.
Boxes 5A 5B 10A 10B Attorney name and address:
In each section for each spouse’s information it asks for the name and address of their Attorney. We are pro se. What should we do with these questions?
Box 23 “Plaintiff:”
Do we write the name of Plaintiff in this box?
Box 24 “Decree Granted To:”
What do we write in this box?
Box 24 “Legal Grounds for Decree”
Our grounds are “irreconcilable differences for 6 months.” The box isn’t large enough. Is it OK to write “irreconcilable differences?”
Box 26 “Signature of person preparing certificate”
Should we cross out “Attorney At Law?
